Waiver and Liability Form
PoleBait X-Pole Stage Pole Rental Waiver and Release of Liability
1. Acknowledgment of Risk: I acknowledge that pole dancing involves risks which may cause various physical injuries. I voluntarily agree to assume all risks associated with participating in any activities involving the XPole Stage rented from PoleBait.
2. Release of Liability: I hereby release PoleBait, Naya Taylor, their affiliates, officers, directors, and employees from any claims, demands, or causes of action that I have or may have in the future arising out of or relating to any loss, injury, or damage, including but not limited to, personal, physical, or mental injury, pain, suffering, or economic loss, incurred in connection with my use of the XPole Stage.
3. Compensation for Damages: I agree to compensate PoleBait for any damages caused to the XPole Stage during my rental period. Compensation will include repairs or, if necessary, full replacement of the XPole Stage to restore it to the condition prior to my rental.
4. Agreement to Follow Rules: I agree to adhere to any and all operating rules and procedures provided by PoleBait regarding the use of the XPole Stage.
5. Confirmation of Understanding: I have read this Waiver and Release of Liability form, fully understand its terms, and understand that I am giving up substantial rights, including my right to sue. I acknowledge that I am signing the agreement freely and voluntarily, and intend by my signature to provide a complete and unconditional release of all liability to the greatest extent allowed by law.
